Symptoms Checklists
A number of different symptoms checklists have been developed to help identify visual development problems that may be keeping you, your child, or a loved one from reaching their full potential. I have culled through many of these and decided to offer a few of these to you. Select the one that most fits your needs and fill it out. The results will come to my email and it will give me a chance to review your situation and send you and email back about what the results indicate. This is offered to you at no-charge and I will gladly share the results with any other eye-care practitioner or health care provide you like. Thank you for taking the time to fill this out.
