pledge the following:

1. To get to you a report of the patient’s findings and to call and relay prescription information to you by phone so that you can provide the patient with appropriate eye-wear in a timely manner.
2. To mark in some appropriate manner your patient’s physical and computer record so that my staff knows that this patient is an optometric referral from you. I pledge to you that we will not see other family members of that patient unless you have specifically referred them or given other permission to see family members. 
3. We agree to submit all necessary insurance claim forms for your patient.
4. We agree to give your patients the highest quality behavioral vision diagnostic and therapeutic services available.
5. We agree to update you on the patient’s progress at key points in their treatment.

I will return your patient to you at the completion of their care.
